Welcome to Amherst College and to Amherst College Athletics! We need to receive certain information from you in order to clear you to participate in your sport. Please reference either the "FOR NEW STUDENTS" link or the "FOR RETURNING STUDENTS" link on the previous page to follow the appropriate clearance procedures. Once completed, please submit all of the necessary information.  Please note, if your health file is not complete, you will not be able to participate in your sport. Below you will find the Pre-participation Physical Exam Form, a Title IX Educational Video and the NCAA Medical Exemption Form.  The Pre-Participation Physical Exam form (as mentioned in the "FOR NEW STUDENTS" link/letter) needs to be completed for all new student-athletes and signed by either an MD or DO.  The Title IX Educational Video must be completed by all students. The NCAA Medical Exemption Form is for any student-athletes currently taking any banned stimulant medications for ADHD and is to be completed by your physician that currently treats you.
